Medical Information
Regardless of whether your student will need to take medication while at camp, all families must complete and turn in an Overnight Field Trip Medication Requirements Checklist.
If your student will need to take prescription or non-prescription medication while at camp (including common over the counter medications such as Tylenol or Melatonin), please review the following information carefully. A Medication Administration Authorization at School form must be completed by your student's health care provider and turned in for EACH medication. If your student will self-carry their medication, please be sure that the accompanying checkbox on the Medication Authorization form is filled out. Also, the school nurse will meet with your student to assess their ability to safely self-carry their medication. Form(s) must be signed by a health care provider. For camp, medication must be in original container, prescription bottle, or package. Only the exact amount of medication (tablet/pills) for length of camp may be brought to camp. Completed form(s) and medication(s) must be turned in to the RSAR office manager no later than 9/5/25. We highly encourage you to schedule an appointment with the school nurse to drop off medications, rather than coming in without an appointment. She can be reached at reharris@lwsd.org.
If you have questions about medications at camp (or at school), please email our School Nurse, Rebecca Harris at reharris@lwsd.org.
For more information, please visit this page: Procedures and appropriate use of prescription and non-prescription medicine
For students with allergies or other medical conditions, additional forms may be required. Please visit this page for more information: Student Health Forms
